8 1/2" x 11" inmate communications form filled out by Jesperson and submitted to staff with their handwritten response on the same page. There are a total of four Jesperson signature variations on this two-sided document.
Also included is an 8 1/2" x 11" copy (Jesperson's) of his active visiting list at the time.  The names on his visiting list appear on the actual copy, but have been redacted in the attached scan.
