John Wayne Gacy Illinois serial killer executed on May 10, 1994.
State Of Illinois Department of Corrections business card for Salvador A. Godinez the Warden of Stateville Correctional Center where Gacy was executed on May 10th, 1994. Gacy signed this card within hours of his execution after he was moved from Menard, IL to Joilet, IL where his execution was carried out.  Not sure why the stains appearance, that's how this was received in 1994.  One of, it not the last full signatures he ever signed.
